Schedule an Appointment
This guide will help you schedule a face-to-face appointment with a provider, to Take a Test within your HealthCheck account. Step-by-step instructions can be found below.
Note: Some features shown in this guide may not appear exactly as shown - and in some cases may be deactivated - due to the features and settings for your particular account.
Take a Test
Open the Dashboard by tapping the three horizontal lines located in the top left corner of the home screen.
Tap Take a Test.
Tap Schedule Appointment.
Note : “Schedule appointment” will be available on the app only if it's enabled for a Tenant
Select the desired test. This will direct you to the Select Facility screen in Step 1, below.
Step 1: Search the Facility
Find the Facility by either entering a city name or a zip code. If the app has access to your location, it will show you the facilities nearby. Otherwise, you can select a facility from the list shown.
Step 2: Select the Facility
Select the facility you would like to visit by tapping on the facility pin on the map, or the name in the list below.
Step 3: Facility Selected
You will see its location on the map.
Step 4: Facility Details
You will be presented with the facility details. Tap SELECT FACILITY.
Step 5: Schedule Appointment
Select the time and date of your appointment and tap CONTINUE.
Note: The “Schedule Appointment” option will be available if it is enabled by an administrator on the Platform Console.
Step 6: Confirm the Appointment
When you tap Continue, the appointment will be confirmed.
Step 7: Appointment : Add to Calendar
You can tap Add to Calendar to add the appointment to the phone’s calendar. If you do not wish to add to calendar, tap Return to Home Screen.
On selecting Add to Calendar, the system will display a notification such as We need access to your calendar to add your appointment to it.
Tap OK to allow access. The appointment will get added to the calendar.
Step 8: Select your Payment Method
Once the facility is selected, confirm how would you like to pay for the selected test.
If you select Use my insurance, you will be redirected to add your insurance screen as discussed in Purchase Test Kit / Enter Insurance Information heading : Step 1 : Add Insurance Information.
If you select Pay with Credit Card, you’ll be redirected to the Checkout page as shown below.
If you have already added your Insurance provider then you will be asked to confirm the Insurance provider details as shown in the below screenshot or update if required.
Tapping LOOKS GOOD will redirect you to the Checkout page.
After confirming the details, agree to the terms and conditions and tap CONFIRM ORDER.
You will be redirected to the Informed Consent screen; read and agree.
Read the HIPAA Consent and agree.
Add your eSignature.
Step 9: Confirm the Appointment
Schedule your appointment.
Tap Add to Calendar to add the appointment.
You can view the test details and the Status on the test order screen.
