Virtual Consult: Questionnaires
Note: Some features shown in this guide may not appear exactly as shown - and in some cases may be deactivated - due to the features and settings for your particular account.
Questionnaires in virtual consult allows you to add questionnaires that employees need to answer from the app.
Create a new Questionnaire
To create a new questionnaire, select Virtual Consult » Questionnaire » NEW QUESTIONNAIRE.
Add a Title in the Overview section. Select Member or Provider from the Questionnaire Type drop-down.
If you want to Allow Comments for this Questionnaire click on the toggle. Once all details are added, Click on SUBMIT.
Click the QUESTIONNAIRE VARIABLES tab and then click ADD NEW VARIABLE.
Enter the Name and Value of the variable and Click SAVE.
All the added questionnaires will be displayed here. The list view will present the Questionnaire title, Created (date and time), Creator, and Modified (date and time).
To edit the questionnaire, select the row which needs to be edited and the page will be opened to make the required changes.
Click on the QUESTIONNAIRE BUILDER tab to add/edit both questions and outcomes that populate the questionnaire. Click Add Questions to add the questions and select from the list of questions.
The Select Questions to Add page will be displayed to select from the list of questions available.
You can also edit and delete the list of questionnaires by clicking on a question.
If you select “Jumps” in a question, you can add multiple nodes. Once all the mandatory details are added, the Save button will be enabled. You can select a Jump Type of Conditional, Non-conditional, or Variable.
If the Jump Type selected is Non-Conditional, then you must select a question to jump to in all cases from a drop-down of questions already selected to be part of the questionnaire. You can add a medical code to associate with the jump including ‘system’ where a value must be selected from a drop-down of CPT (Current Procedural Terminology), ICD (International Classification of Diseases), LOINC (Logical Observation Identifiers Names and Codes), Master Specimen, Modifiers, and NDC. A code description and code can also be added.
You can also add other navigation links multiple times where you add the administrative short name, text to display and a link destination.
If you select Jump Type as Variable, enter the details in Variable Name, Operation, Answer, and Then Jump To values.
When viewing an Outcome, you can set additional Jump Types: Variable, Conditional, Non-conditional and Score.
Once Questions and Outcomes are successfully set up, a preview can be viewed in the Questionnaire Preview tab. This will give an idea on how the questions and outcome will be displayed in the app.
