The “My Health Record” section enables you to store all personal health-related information. The data can either be imported from a health provider or entered manually.

To access this section, tap on the navigation menu on the top right of the screen and then tap My Health Record.

The My Health Record screen enables you to store health-related information under the following categories:

• Allergies

• Conditions

• Demographic Info

• Family Details

• Medications

• Procedure

• Social History

• Vaccinations

• Vitals

When you tap on a particular category, the app will display the available options for entering the information.

For example, when you tap on the Conditions category, you can import the data from a health provider by tapping the Import Health Records button or enter it manually by tapping Add Condition button. Please note the options to enter the information will vary depending on the category selected.

Tapping Add Condition will allow you to enter the following information:

Enter all the required information on the page.

You can also add a Note to provide additional information related to a category, if required. To add a note, type the information in the Notes field and tap the green arrow button to save the note.

The saved note will be displayed in the “Notes” section; you can add multiple notes by following the above step, if required. You can delete a note by tapping the X icon in the field.

After you have entered all the required information, tap the Save button to save the record.

The entered information will be stored and displayed in the respective category, which is “Allergies” in this case.

The information can be edited by tapping on the record.

You can add additional data by tapping Add.
