Health Record: ID Verification

Please note: some features shown in this guide may not appear exactly as shown - and in some cases may be deactivated - due to the features and settings for your particular account.

For privacy purpose, we need to verify your identity as we will be importing your health records from the electronic health record systems. This section shows the process to verify your ID in order to better serve you.

Step 1 : Verify your ID

In order to verify your details, you need to provide Government issued document. You can select any of the following to proceed further.

Step 2 : Scan the Document

Scan the document as per the required criteria.

Step 3 : Verify Quality of the Scanned Image

After capturing the document, please verify if the captured image is clear & in readable form.

Step 4 : Adjust the Image

Adjust the image as per the window and upload the document.

Step 5 : Add your Phone Number

Enter your phone number which will be connected with the account.

Step 6 : Verify your Phone Number

You will receive a 4 digit code on your number which will be used to verify your number. Please use the number that is active & in use to avoid any inconvenience.

Step 7 : Add your Email Address

Enter your email so that you can access your account using the email. You can change it later on if you like

Notification message will be displayed once the email is verified

Step 8 : Read & Agree with the HIPAA Consent

HIPAA(Health Insurance Portability and Accountability Act) is applied when dealing with the health related information. Please read the HIPAA Consent Carefully and agree with the terms & conditions mentioned in order to further continue.

Step 9 : eSignature

Confirm your consent by signing in the app and Tap on “DONE” button.
