Vaccination Records
This user guide covers the steps and instructions needed to verify that you have been vaccinated (or exempted). Step-by-step instructions can be found below.
Note: Some features shown in this guide may not appear exactly as shown - and in some cases may be deactivated - due to the features and settings for your particular account!
Verify Vaccination
Follow the steps below to upload this information:
1. Tap the profile icon in the top right corner of the screen to access the navigation menu.
2. Tap Vaccination Record.
You will be taken to the Vaccination Records page.
3. Tap VERIFY VACCINATION RECORD to proceed to the next step.
4. On the next page, tap one of the available options:

Scan a Credential
1. Position your phone’s camera to scan the credential.
2. Tap CONFIRM INFORMATION.
3. If the QR code is not valid, you will receive the following message:
4. Complete the form, and then tap SEND.
5. If the code scanned properly, you will see the message shown below. Tap CONTINUE.
You will be taken to the Vaccination Details page to confirm the information is correct.
Import a Credential
Select how you would like to import your credential:
Apple Health
Google Pay
Import from Photo Library
Select a QR code from the Photo Library.
Tap Choose.
The Vaccination record is imported. Tap Continue to Confirm.
Record is successfully updated.
You can view the added vaccination details after tapping Continue.
Import from My Files
Verify via Patient Portal
1. Select your patient portal from the list of Providers. If your provider is not visible, enter the name of the provider in the Search Providers field at the top of the screen.
2. When you have located your provider, tap SYNC.
3. Enter your Username and Password for the patient portal to log in. Tap Authorize data access.
4. Check Email me a copy of this authorization if you desire a record of this data access.
5. Tap AUTHORIZE to complete the process, or DENY to abort.
Confirm with Provider
1. Select your provider from the list, or use the Search Providers field if you don’t see it.
2. Agree to the HIPAA Consent form.
3. Tap Done.
Attestation of Vaccination/Exemption
Follow the steps below to upload this information:
1. On the Verify Vaccination page, tap the Attestation of Vaccination option.
2. The app will display a preview of the photo taken. If the image taken is clear, tap Submit to proceed to the next step. If an image is not clear or needs to be retaken for any reason, tap Rescan to capture the photo again.
3. Tapping Submit will take you to the page shown below to enter the vaccination information. This form allows user to fill in information about the first and second doses of the vaccine. User can upload multiple vaccination records. You can add up to 3 pictures per each vaccination record and that can be added one by one.
4. You will also be able to enter information related to the booster doses if needed. To add, tap Add Booster, and the relevant fields will be displayed. You will be able to add the information for multiple booster doses.
5. Details of doses needs to be added in chronological order. Date of Dose 2 must be greater than Dose 1 date. If the date of Dose 2 is not greater, the system will display the message Date of Dose 2 must be later than previous dosage.
6. To delete information related to a booster dose, tap the delete icon as shown to the right.
7. After entering all the relevant information, tap CONTINUE to proceed to the next step.
8. The Attestation screen will be displayed to confirm that the photo of the vaccination card belongs to you. Tap CONFIRM ATTESTATION to proceed to the next step.
9. A confirmation message will be displayed indicating that the details have been submitted successfully.
After tapping the “Done“ button, added record will be shown.
10. You can delete the record by tapping the Remove button, which is located in the corner of the thumbnail, if needed.
If you delete the last record, you will be prompted to scan a new one.
Note: Tenant Admins have the autonomy to reject submissions if the images aren’t clear or appear invalid, etc. If this happens, you will have to resubmit new images.
Upload Exemption Card Information
1. Tapping Exemption Card will take you to the screen to capture the photo of the exemption card. Place the card on a flat surface and capture the photo by tapping the round capture button.
2. The app will display a preview of the photo taken. If the image taken is clear, tap LOOKS GOOD to proceed to the next step. If the image is not clear or needs to be retaken for any reason, tap RESCAN to capture the photo again.
3. Tapping LOOKS GOOD will save the exemption card details. You can also tap Remove Exemption Record to delete the record.
Note: Tenant Admins have the autonomy to reject submissions if the images aren’t clear or appear invalid, etc. If this happens, you will have to resubmit new images.
From here, your organization’s administrator will update your record to reflect your vaccination status or exemption.
