Take a Test: Test Kit Registration and Collection
Note: Some features shown in this guide may not appear exactly as shown - and in some cases may be deactivated - due to the features and settings for your particular account.
This guide will help you to register a test kit, Take a Test, collect and capture the test sample, and get your test results within your HealthCheck app. Step-by-step instructions can be found below.
Register Test Kit
If you have already ordered and received a Test Kit, it can be registered by following any of the three options listed below:
Option 1

Tap the three lines in the top left corner of the screen to access the navigation menu.

Tap Test History.

Tap on the order.

Tap Register Kit (Scan Code).
Option 2

Tap the three lines in the top left corner of the screen to access the navigation menu.

Tap Take a Test.

Tap Register a Test Kit.
Option 3

Tap Take a Test on the Home screen.

Tap Register a Test Kit.
Scan QR Code
Depending on the type of test kit that is ordered, follow the instructions provided in the test kit to locate the QR to scan. Register the test kit by tapping on SCAN QR CODE to scan the QR code then proceed to the Collection Instructions.
Registration can be successful or unsuccessful.

Registration means the test is considered to be "used" from that point forward since serial number has been detected.

If you do not not actually take test at that point in time and try to scan the same cassette in the future, it will throw a "test has been used" error since it will detect a used serial number from before.

Errors when Scanning QR Code
Scan the QR code to register the test kit. If the scan is unsuccessful, the member will see one of the below error messages:

If the Test has been used:

If you scan an expired test , you will be taken to the IFU’s without being prompted an error message regarding test expiration as shown below:

If the test has been recalled:

If the QR Code is unsupported:
Errors should allow you to rescan or navigate back to home page.
Test Kit Confirmation

If the QR code is scanned successfully, you will see an image of the test kit along with its contents. Verify the Test kit package to confirm you have the correct kit. Tap CONTINUE to get started.
Please note that in some cases, the contents may not match exactly the image due to changes in package design. If you have any questions, reach out to your institution to confirm that you have the correct kit before proceeding.

When you logged into the app and scanned the test kit and ready to start the collection instructions, the app verifies if the possibility to skip the instructions is available. Skipping of collection instructions is available when below conditions are met (Both conditions should met at the same time).
Test Kit configured to skip instructions, AND
Tenant configured to support skipping instructions, AND

The Proctor feature is OFF for the specific Tenant

Collecting Test Sample and Test Results
Collection Instructions
Read and follow the collection instructions and make sure that the test sample is prepared correctly. Your instructions will vary, depending on the exact test you are taking. Follow the instructions carefully to ensure the test is taken correctly.

Wash your hands. Make sure they are dry before starting.

Collect First Sample.

Apply 4 Drops.

Wait for 15 minutes.

Scan Test Results.
Scanning Test Result

The app will provide instructions to capture the image correctly. You can rescan as many times as required within the allotted time.

If the test kit is placed correctly, the guides will turn green to show that you can now capture the image.

If the test kit is place incorrectly, the guides will turn yellow to show that you cannot capture the image yet until it is correctly aligned.

Once Captured, it will display a message that image is good or if it needs to be rescan.

The system will display the time left to complete the scan.

If you fail to scan the test cassette, you will be shown an expiration message at 0 seconds: The time to complete the test has expired. This test result is Invalid.

Errors when Scanning a Test Cassette

Once the image is captured, the app will process the image and if there are any issues with the image captured, system will display the failure messages as listed below. You can recapture the image by tapping Try Again.

Test Completion and Test Results

As a Member, you want to know when the app is processing information, so that you are not confused as to why are you waiting.

If the image is processed correctly, then the following screen will be displayed. Tap CONTINUE to see the test results.

The Test Result could be either Positive, Negative, or Inconclusive.
You can view the Test Cassette image by tapping on Test Cassette

Choose a Test
As a Member, you want to see visual representations (tiles) of tests so that you can compare the test kit you have to the ones you see on your screen and ensure that you are choosing the correct one.
If the tenant has multiple test kits active, “Choose a Test” screen will be shown once you select I Have a Test. Note: Choose a test screen will be shown if enabled in the tenant settings and configured in the orderable lab test.
Tap on Register a Test kit to go to Choose a Test screen
Test Scenario
If the tenant has 1 test kit active, “choose a test” screen will not be shown.
Tap on Register a Test kit
“Choose a Test” Feature Turned Off Scenario
If the tenant has “Choose a Test” Feature Turned Off, a default place holder image will be shown.
Multiple Test Scenario
If the tenant has multiple test kits active, “Choose a Test” screen will be shown.
Tap on Register a Test Kit
If you scan a test kit that they did not select (e.g., you select Flowflex and then you scan BinaxNOW test kit).
If a correct code is scanned, you will proceed to the collection instructions as shown previously in this page.
Choose a Test Specs
As a member, you want to select the test you wish to take by searching for the name.
You can search by the Manufacturer & Test name. You will see the results narrowed down by the search as you type.
If you search for something that cannot be matched, you will see the default message: “Sorry! No test found. Please try another test name.”
You can scroll down to find the required test.
App Dashboard Passport Card

The Passport Card on the Home Screen Dashboard will also be updated based on the test result.

If the result is Positive, the color will be red.

If the result is Negative, the color will be green.
-
If the result is Inconclusive, the color will be grey.

Users can self access or self report their test result using the Self Assessment feature discussed here.
Downloading Lab Report

Tap the three lines in the top left corner of the screen to access the navigation menu.

Tap Test History.

Tap the Test to view the details.
-
Tap Download Lab Report to download the test report.

The test result will be downloaded in the PDF format as shown in the below image. You can also view the picture of Test Cassette in the pdf file.
