Signing Up and Account Activation Process
This guide is an overview of the steps and instructions needed to help you get your HealthCheck account set up. Here you will learn how to set up your account, password, add account information, and how to set your account security passcodes.
Note: Some features shown in this guide may not appear exactly as shown - and in some cases may be deactivated - due to the features and settings for your particular account.
After downloading the app, click on the app icon to open it; a splash screen will appear, as shown below, and you will then be taken to the introduction screen.
The Introduction screens, similar to the ones below, are shown only when the app is opened for the first time after installation. They may have some basic information about the application before you get started.
NOTE: These screens may not exactly match your experience: your institution may modify the messaging and images, or turn them off entirely. If the Introduction Screens are not displayed, you will be taken to the Login Screen directly. You can click on the Sign up link on this screen to initiate the process.
If you don’t have an existing account, you may sign-up for a new one by following the steps below:
On the Introduction screens, tap the Sign Up button and you will be taken to the Sign up screen.
Enter the information requested on this screen. You will be asked to provide the following information (all the fields are required):
Email (must not currently exist in the system)
If the entered email address exists in the system, you will see an error informing you that you can sign in with this account.
After entering a unique and valid email address, tap the Sign up button.
Email Address Verification
You will be taken to the next step to verify your email and you will receive an email with the verification code.
Enter the verification code in the app and then tap on the Continue button. If the code is valid, you will be taken to the next screen to set a password for your account.
Create a Password
Create a password by following the instructions shown on the page:
The Password must contain:
at least 12 characters
1 Lower case letter
1 Upper case letter
1 Special character (!@#$%^&*-)
You have the option to enable Biometric ID instead of password for future logins
You must accept Terms & Conditions and age attestation to complete the account creation
If the Terms and Conditions are not accepted, the button to create the account is not enabled
You can view Terms and conditions by tapping on the link “Terms and Conditions“ in the screenshot below:
If the password entered in the 2 fields does not match, you will see the following message displayed:
If the password does not meet any of the required criteria mentioned on the screen, the failed criteria will not be marked in green as shown below.
After a entering the valid password and tapping the checkbox for the acceptance of the Terms and Conditions, tap on the Submit button to proceed.
Phone Number Verification
You will be navigated to the Verify Phone Number screen, enter your mobile number and tap on the Continue button.
You will get a text message with a code on the phone number you entered. Enter the code and tap the Continue button.
If the code is incorrect, you will be shown the following error screen.
Close the error screen to re-enter the code. You can also click on the Resend Code button to receive a new code.
When you click on the Resend Code button, you will be informed on the screen when a code is resent.
Click on the Continue button after entering the correct code to navigate to the next screen
Add Member Information
Based on the settings configured by your administrator of your organization, you might see the next screen to capture additional information. The fields on this screen will vary but will be similar to the sample screenshot below:
After entering the information on this screen, tap on the Continue button to navigate to the next screen.
Verify Shipping Address
Enter your shipping address or validate the address if it’s prepopulated and make the corrections if needed and tap on the “Continue” button .
If the administrator has enabled the setting to verify the shipping address, then the address will be verified by a third-party system, like USPS. You will be prompted to verify the address that was validated. Choose the desired address and then tap on the Confirm Address button to navigate to the next screen.
If the address entered is not correct, you will see the following message displayed on the screen. Tap the Try Again to re-enter the address or tap the Proceed Anyway button to continue with the address entered.
Setting Up a Passcode
You will be prompted to create a passcode on the next screen by entering a 4 digit number. This is an optional step and if you don’t want to set a passcode at this time, tap the Cancel button at the bottom of the screen.
If you enter a passcode, you will be prompted to re-enter it again in the next screen.
Welcome Screen and Dashboard
After you have reconfirmed your passcode, you may see a welcome screen similar to the sample screenshot below. This screen may or may not be available for your organization based on the configurations by your administrator. You can tap on the I’ll do it later button, if you don’t want to take up the Virtual Consult.
Your account is now setup and you taken to the app dashboard.
You might be asked to agree to the Terms and Conditions. Tap on the Accept or Agree button at the bottom of the screen to return to the dashboard. Sample screenshot below:
