Symptom Checker: Virtual Consult Questionnaire
This guide is an overview of the steps and instructions needed to help you check your symptoms in the HealthCheck app. Here you will learn how to check your symptoms by using the Virtual Consult Questionnaire. Step-by-step instructions can be found below.
Note: Some features shown in this guide may not appear exactly as shown - and in some cases may be deactivated - due to the features and settings for your particular account.
This section allows you to check your symptoms before you visit your institution (e.g., report to work, go to school) to avoid the spread of the disease.
Begin a Questionnaire
Check your Symptoms: Tap Begin Consultation in order to check your symptoms.
Virtual Consult: Tap Start Virtual Consult to start the Symptom Checker.
Answer the Questions: Answer the list of questions presented.
NOTE: Your list of questions may vary depending on your institution and the local guidelines in your state/region.Select any of the symptoms that you're experiencing, then tap CONTINUE at the bottom of each set of options.
Below are the screens where you can add responses
List Response
Tap on “+“ icon to add a response
List all current medications you are taking by typing and selecting from the list.
Added medications will be listed. You can add more if you want.

Tap on “Enter Your Profession“ to start writing your response
Response is added

Below is the field where you are required to input number

Select the option from the dropdown

Search List Response
Type in the search box to add a response
Edit List Response
Tap on the option to edit in order to make changes or if everything looks correct, please continue.

Prescription Eligibility
In order to check the prescription eligibility, take virtual consult as shown below.
Outcomes
Member has completed the Virtual Consult after testing positive to ascertain if and what treatment is appropriate.
Given that the member has carried out the prescription eligibility, when the member answers the questions in a certain way, then they are offered a prescription.
If you have answered the VC in such a way that it is recommended you take a test, you will be presented with three options : I Have a Test, Order a Test, and Find a Local Retailer.
If you have answered the VC in a way that prompts the clinician to seek a Telehealth consultation:
Given that the Outcome is a Telehealth conversation, when you are presented with this info, then you are able to select ‘begin call’ or ‘more info’.
If you have taken the VC and are cleared, you are able to select ‘take a test’ or ‘return home’.
Restart or Resume
As a member, you want to be able to resume a previously started VC, so that you can continue the data entry you left off.
When there is a pre-existing VC, you can choose to resume it.
When there is a pre-existing VC, and you try to start a new one, you can choose to resume the old one or re-start.
When you choose to resume the previous one, then the app will resume the previous one.
If there is a pre-existing VC, and you choose to start a new one, then the app will start a new VC and discard the old one.
