Virtual Consult Questionnaire
This guide is an overview of the steps and instructions needed to help you check your symptoms in the HealthCheck app. Here you will learn how to check your symptoms by using the Virtual Consult Questionnaire. Step-by-step instructions can be found below.
Note: Some features shown in this guide may not appear exactly as shown - and in some cases may be deactivated - due to the features and settings for your particular account.
This section allows you to check your symptoms before you visit your institution (e.g., report to work, go to school) to avoid the spread of the disease.
Navigating the Questionnaire
Tap Begin Consultation to navigate to the questionnaire
Tap Start Virtual Consult to start the questionnaire.
Answer the questions as presented and tap the CONTINUE button at the bottom of the screen to navigate to the next question.
NOTE: Your list of questions may vary depending on your institution and the local guidelines in your state/region.You can tap back button on the top left to return to the previous question and change the answer if you want.
If you want to exit the questionnaire either to continue it later or stop taking it, you can tap the X on the top right corner of screen. You will see the following notification on the dashboard screen and the Virtual Consult dashboard card will not be available. You can tap on the Cancel button to stop taking the questionnaire
Tap the Continue button to resume the questionnaire from where you left off.
Types of Responses
The questions in the questionnaire might be configured to accept different types of answers or responses. Below are some of them which you might see:
List Response
When a question has a response type as a List, you can tap on + icon to search for an option from the list and add it as an answer/response. Sample screenshots below.
You can choose multiple answers by clicking on the + icon or click on the X beside the added option, to remove it.
Text Field
Questions can also be configured to accept answers in a text field. You can input any thing in this field. Sample screenshots below.
Numeric Response
A question can be configured to accept a numeric answer. You will be presented with a number pad to input your response. Sample screenshot below:
Dropdown Response
A Dropdown response type provides a list of options that you can choose from. You will be able to select only one of the listed options. Sample screenshot below.
Search List Response
The Search List response type provides a search box along with a list of options that you can select. The search box can be used to search for an option listed. Sample screenshot below.
Yes/No Response
A question can also be configured to receive a Yes / No response. You will be able to choose either one of the options provided.
